At the time an individual applies for the Child Care Assistance Program (CCAP) or begins participation in an approved educational activity during the certificate period, verification of school attendance must be obtained.
The following items are required to be included in the case file:
- A copy of the class schedule for each semester
- SFN 113 Postsecondary Education Information Form
- If needed to verify the degree being pursued a copy of the curriculum is required
